A new report says a B-1 crash at Ellsworth Air Force Base was caused by a combination of weather conditions and mistakes by the crew. The Air Force Global Strike Command performed the investigation.

The Defense Department is set to review the Medals of Honor given out to U.S. soldiers after the 1890 Wounded Knee Massacre. An estimated 250 Lakota men, women, and children were killed. Twenty U.S. soldiers were given Medals of Honor following the event.
